Important Information

Pregnancies that begin within 90 days of Membership with an estimated date of delivery within the first year are not shareable. For example, if your Membership is effective October 1st, to be eligible for sharing your estimated delivery date must fall after October 1st of the next year.

Pregnancies that begin on or after January 1, 2026 are subject to a $5,000 Initial Unshareable Amount (IUA) before any costs may be shared.

Tobacco Guidelines

Sharing is limited for tobacco or smoked product users 50 years of age and older.

Sharing is limited to $25,000 for each of the following categories: stroke, cancer, chronic obstructive pulmonary disease (COPD), oral and esophageal disease, and gastric and duodenal ulcers.
